Assessment and Services
We provide a comprehensive at-home assessment that takes into account the abilities of the client as well as those of the family and support network. We help you identify opportunities and resources by presenting you with our findings in a formal plan of care.
We also offer you the option for us to:

●Identify familial roles and mediate between caregivers when consensus cannot be reached.
●Help apply for, utilize and appeal public entitlements and private insurance benefits.
●Coordinate and oversee in-home care and services.
●Access free or low-cost community resources where available.
●Facilitate housing transition and placement
●Assist with paying bills and resolving discrepancies
●Schedule and attend medical appointments.
●Liaison between providers, clients and long-distance caregivers
●Inventory financial and legal information and documents.
●Make appropriate referrals to other agencies and professionals when necessary.
