As Geriatric Care Managers we guide, advocate for, and provide resources to seniors and their caregivers. This allows the older adult the opportunity to reach his or her maximum functional potential and quality of life.
Based on a professional assessment, Life Management Resources works with the older adult and his or her loved ones to develop a plan of care and services that promotes autonomy and independence.
